For New Student

  1. Grade 12 Report Card (Form 138)
  2. Certificate of Good Moral Character
  3. PSA Birth Certificate (original and photocopy)
  4. 2 pcs. 2”x2” and 2 pcs. 1”x1” ID Picture with name tag
  5. TAU Admission Test

For Transferee

  1. Honorable Dismissal or Transfer Credential
  2. Transcript of Records (TOR)
  3. Certificate of Good Moral Character
  4. PSA Birth Certificate (original and photocopy)
  5. 6 pcs. 2”x2” ID picture with name tag
  6. Validation Test for courses earned in private HEIs

For Foreign Student

  1. Personal History Data
  2. Student Visa/ Study Permit
  3. Alien Certificate of Registration
  4. Proof of Citizenship
